All that remains today of the payphone that once stood at 31-08 Astoria Boulevard South is a set of 4 metal spikes and this photograph I left in memoriam.
Stuck among a gas station, a real estate office, the Grand Central Parkway, and a Greek dance club, this photo shows that this payphone stood at your service as recently as November, 2009:

The word “POLIVIO” was hand-written on the top left of the front of this payphone enclosure. I do not know if that word has any significance related to this particular payphone or its location on Astoria Boulevard near the Grand Central Parkway. I thought it odd enough that I shall call this the Polivio Payphone.
I don’t know when this payphone was removed. Today only 4 rock-hard nipple-like metal spikes (and a box of lipstick) remain to prove a payphone once stood here:
